Lay Leaders to Serve People with Chronic Diseases

A graduating class of excited, skilled and compassionate individuals recently completed their lay leader certification in Stanford University’s Chronic Disease Self-Management Program (CDSMP).   The CDSMP addresses many chronic illnesses (i.e. arthritis, asthma, obesity, hypertension, pain, heart disease), including diabetes.  The lay leaders will offer an array of subjects matter in a six-week, 2 ½ hour long program and complement our current Diabetes Self-Management Program (DSMP), and serve more people affected by a number of chronic health issues.  Open to participants throughout Dallas County ages 60 and older with one or more chronic health condition.   For more information, please contact Jessica Walker at 214-954-4218.
