SCOPE OF SERVICES Deaf Action Center


Service Description

The project shall provide information, referrals, and assistance to allow access to a wide array of services for elderly persons who are deaf or hard of hearing. The purpose of the project is to assist persons to maintain their independence and self-sufficiency by providing opportunities to bridge communication barriers and reduce isolation. The project shall provide information about other programs and appropriate services/technologies. The project will actively participate in linking the inquirer to needed services.

Project shall also conduct follow up on referrals to ensure that the services/technologies were received or provided.

  • Provide an overview of your system for determining appropriate inquiries for and making referrals to the Dallas Area Agency on Aging.

  • Explain how an IR&A request is tracked, from initial inquiry through follow-up contacts, to determine whether the caller’s needs were met and services accessed. Include a description of how the determination for a follow-up contact is made

Unit of Service: One contact - record one contact for every communication with or on the behalf of an eligible individual, regardless of the type of contact (initial, follow-up, accessing services). Counts of initial and follow-up contacts must be collected and reported separately.
