Home Delivered Meals

This programs works in conjunction with the Visiting Nurse Association (VNA) by providing activities that directly relate to the initial assessment and required reassessment of Title III home delivered meals participants.

Eligibility Criteria:

Client is homebound individual 60 years of age or older

Clients must reside in Dallas County

Score 20 or higher on the Texas Department of Aging and Disability Client Needs Assessment Questionnaire (Form 2060).

Score 6 or higher on the Nutritional Risk Assessment Form.

Referral Process:

• Clients may be referred to this program only by the Visiting Nurse Association and/or Advocacy Center for the Elderly Network Participants.  Click here for a listing of Network Participants
